Residential and Community Based Rehabilitation

The Recovery Project offers residential and community-based rehabilitation. This program utilizes evidence-based therapies combined with the creative use of a patient’s own surroundings, whether at home, school, work or in their community.

Receiving rehab services in the home and community can address goals and barriers in real life settings and optimize an individual’s quality of life.

Home and community rehab therapy services are best suited for adults who:

  • Are home-bound and/or have a medically complex diagnosis and needs
  • Have transportation difficulties that limit access to the community
  • Have specific functional goals directly related to their home or community environment
  • Require transitional therapy services between inpatient and outpatient care
  • Have durable medical equipment needs

Services offered include:

  • Physical therapy
  • Occupational therapy
  • Speech therapy
  • Massage therapy
  • Recreational therapy
  • Fitness training

If circumstances have prevented you from coming into our clinics, please call us at 855 877 1944 to see if you are eligible for home services. Home and community-based rehabilitation services are currently available to individuals with no-fault auto insurance and Blue Cross Blue Shield Michigan Community Blue PPO plans (some exclusions may apply).
